Requirements For Participation In NCAA Sports At WCSU

 

For All New Players

(Including Freshmen, Transfer Students and Upperclassmen Who Have Not Previously Played).

The following forms must be completed and returned to the Health Service Office prior to your participation in any varsity sports, including any sports’ camps.  Failure to submit any of these forms will delay your clearance for participation.

Parts A, B, C and D are to be completed by the student. Your health care provider must complete and sign part D. (See complete instructions on the form).

  • A Pre-participation Physical Examination Form For Varsity Athletes must
    be completed and signed by your health care provider. This form must be completed in addition to the above
    CSU Health Service Confidential Health Form if you are participating in varsity sports.  The pre-participation physical exam must be done after March 1st of the current year and only needs to be done once unless medically indicated otherwise.
     
  • An Authorization For Release of Information Reciprocal Disclosure Form must be signed.
     
  • A Questionnaire For Participation In Varsity Sports must be completed and signed. All questions must be answered completely.
     
  • If you have any history of a heart murmur, irregular heart beat, or other
    cardia
    c problems, an echocardiogram report or a note from your cardiologist
    or physician clearing you to participate in varsity must be enclosed.
     
  • If you have had a recent injury, surgery or serious illness or are currently under a physician's care for any of these conditions, a note from the treating physician clearing you for varsity sports must also be enclosed.

Returning Players' Requirements

  • All athletes who have been previously cleared for varsity sports must complete an Athletic Questionnaire and have their blood pressure and pulse recorded for each academic year that they play. 

  • Athletes may make an appointment at Health Service or may have the athletic trainers record their blood pressure and pulse. All questionnaires are to be filled out completely including the months and years of any illnesses or injuries listed. A signature and date are required at the bottom of the form.

  • Any injuries or serious illnesses treated by a private physician will require a note from the treating physician stating that the athlete is cleared to participate in sports. A copy of this must be must be sent to Health Service and will be filed in the athlete’s chart.

  • Each player is responsible for reporting any changes in his / her  medical history to Athletics and the Health Service between the spring review date and the start of training camp.

Coaches, trainers and athletes should be aware that athletes will not be cleared to participate in any varsity sport until all of the above requirements are completed and have been reviewed by the Athletic Coordinator.
